Observed: Schedule Now / Book Online opens Housecall Pro, starting with street address, city, state and ZIP verification. The homepage contact form sits below several content sections and includes name, phone, email, customer/inquiry controls, referral selection and opt-in.
Recommendation: Test a prominent callback/service-request form using service, name and phone. Ask for additional information only when operationally needed; retain calling and full booking.
Revved hero-form concept
Observed: The inspected Revved concept places service selection in the hero. Its four-step flow covers service, urgency and property, then requires name, phone, email and address/city.
Recommendation: Verify testimonials, offers, phone numbers and availability claims. Measure qualified leads and booked jobs per eligible session, step abandonment, spam and response time.
Limitation & Scope: PDF pp. 39–40, 48. Claims about seven required inputs, measured mobile abandonment, live calendar availability or a proven conversion lift have been removed. Screenshot assets not supplied — no screenshots fabricated.
